Directions for those who wish to print out and fill in the Quilt History form to bring to Documentation Day:

1. Please PRINT legibly.

2. In the upper left corner of the first page there is a vertical box that says: Documenter.  Please enter “Owner documented” in that space.

3. Do not write in the margins.

4. If you have more information than can fit on a line, draw an arrow to the edge of the page and continue on the back side, being sure to include the number of the line you are expanding on. Example:  for item number 6, just write 6. and continue writing as you wish.

5. The form is designed to be comprehensive and you will probably not be able to answer all questions. Simply fill in all history and information you do have.
